- Biographical or Historical Note Dr. Shaw's practice was located at 5620 West 63rd Street in Chicago (1923-1924 Chicago Medical Directory) and later at 5610 West 63rd Street (1930-1932 & 1943 Chicago Medical Blue Book). Dr. Shaw graduated from Rush Medical College in Chicago and was a physician in general practice.
